The H2E forms are available in Microsoft Word for 98 or higher.

How to download and use H2E Forms

To download an H2E Word form, Right click on the name of the form. Choose “Save target as” (or "Save as") from the menu that appears, select a folder on your hard drive and save the file to it.

If you left click on the form and it opens, remember this is only a web version of the document—you will still need to save it to your hard drive. Go to the "File" menu at the top of your browser window, choose "Save as", select a folder on your hard drive and save the file to it. (Doing this as soon as you open the form will avoid disasters like losing a completed form because you thought it was on your computer but it wasn’t!)

The file will appear as a document template (with the file extension “.dot”) but you can work with it as you would any other word processing document. Open the file in Word, add the requested information, and save the file—it will save as an ordinary Word document (file extension “.doc”).

You will notice that you can only type in the gray areas within the form. These are the fields where you are expected to put your information. The gray boxes will expand to accommodate any and all information you wish to include. The non-gray text of the H2E form is locked to maintain consist formatting for the overall document. You can move from field to field by clicking on fields or by using your “tab” key. You can print the form anytime you desire.

Click the Back button at the top of your browser window now to return to the page from which you wish to download a form.

Some of the files on the H2E web site require a software application for viewing or use. Below is a list of the required applications, with links to an offsite source for the application that will open in a new window:
Download Adobe Acrobat Reader
Acrobat Reader is required to read Portable Document Format (PDF) files. Download Acrobat Reader free from the Adobe website.
Download QuickTime Player
QuickTime Player is one software package with which to view video files with the "MOV" extension. Download QuickTime Player free from the QuickTime website.
Download QuickTime Player
Windows Media Player is one software package with which to view video files with the "WMV" extension. Download Windows Media Player free from the Microsoft website.
Download Microsoft Word Viewer
Microsoft Word Viewer is required to read some files with the "DOC" extension. Download Word Viewer free from the Microsoft website. (A commercial version of Microsoft Word, or a word processor that can open Microsoft Word files, is required to modify a "DOC" file.)
Download Microsoft Excel Viewer
Microsoft Excel Viewer is required to read files with the "XLS" extension. Download Excel Viewer free from the Microsoft website. (Some spreadsheet features may only work with the commercial version of Excel.)
Download Microsoft PowerPoint Viewer
Microsoft PowerPoint Viewer is required to read files with the "PPT" and "POT" extensions. Download PowerPoint Viewer free from the Microsoft website.
Download WinZip Evaluation Copy
WinZip is required to open files with the "ZIP" extension. Download a free evaluation version of WinZip from the Winzip website.
